Requirements for Application

4. Child’s Birth Certificate/Singapore Permanent Resident Re-entry Document (Photocopy)
   
   
5. Psychological Report (completed within two years from the date of referral).
   
   The report must include the followings:
   
   
   1. Results of the child’s individually administered intellectual assessment that present an overview of the child’s general cognitive/intellectual functioning. The results should include verbal, non-verbal and global scores and subtest scaled scores. If specific IQ scores are not obtained, reasons should be provided and any alternative assessments should be documented. Language(s) used during testing is (are) to be indicated.
      
      
   2. Results of the child’s adaptive/social functioning as measured by an adaptive behaviour assessment. The child must have significant limitations in at least two of the following areas: communication, self-care, home-living, socialization skills, use of community facilities, self-direction, health and safety, functional academics and leisure and work.
      
      
   3. A summary of the child’s individual educational needs.
   
   
6. Where applicable, the application should also include a report from the child’s current class teacher, previous school reports and copies of examination scores.
   
   
7. Where applicable, the application should also include progress reports and recommendations from professional staff working with the student (e.g. Speech Therapist, Occupational Therapist).
   
   
8. The child needs to be at least 5 ½ years of age at the time of school intake assessment.
